Service contact information
Service detail
The Cheshire and Merseyside Rehabilitation Network (CMRN) is the largest national collaborative partnership between provider and commissioning organisations to deliver specialist rehabilitation services across a pathway from hyper-acute to community services. The network members include NHS, independent, social services and voluntary sector organisations.
Inpatient and community services
The CMRN works collaboratively with six provider partner organisations and multiple commissioning organisations resulting in a comprehensive model of specialist rehabilitation care to provide supportive, active and extended specialist rehabilitation to adults following traumatic injury or illness. One of the unique aspects of the service is that provision is based on patient need as opposed to diagnosis.
Our inpatient and community services are delivered by highly skilled interdisciplinary teams who work in partnership with patient and families to achieve a person-centred approach to specialist rehabilitation.
Rehabilitation
Rehabilitation may be offered at different stages of the patients journey. Our service provision extends from early post-ITU rehabilitation through to community-based rehabilitation services. The location of where and when rehabilitation is offered is based on the patients need at any given time.
